RESUMEN

OBJECTIVES: Provide an overview of navigation in three disparate populations: rural, Native American/Alaska Native, and low- to middle-income countries. Discuss gaps in care and opportunities to improve cancer care. METHODS: A literature search was conducted in PubMed and on Google Scholar using search terms, nurse navigation, cancer, disparit*, low- to middle-income countries, Native American, American Indian, and rural. Peer-reviewed research studies, review articles, databases and websites of professional organizations, and historical books were reviewed to provide an overview of oncology nurse navigation in underserved communities. Experiences in working with these populations over the past 30 years were also provided to support current literature. RESULTS: Forty references were included in this overview of nurse navigation in underserved communities. Nurse navigation in these disparate areas is in its infancy. While some programs exist and outcomes have been positive, their dissemination is sparse. A need exists to expand nurse navigation into these areas to provide care for these underserved communities. CONCLUSION: Oncology nursing navigation for each of these underserved communities requires a culturally sensitive approach. Many of these approaches are universal to cultural competency and can be applied to most disparate populations. IMPLICATIONS FOR NURSING PRACTICE: Nurses comprise the largest workforce around the globe and are well-equipped to develop navigation programs in some of the most disparate communities around the world. To do so, it is important to use a foundation of building trust, embracing individual differences, providing culturally sensitive education and resources for growth, and good communication.

RESUMEN

Sleep-wake disturbances are common in patients with cancer. Despite the high prevalence of altered sleep patterns in oncology settings, there remains a gap in consistent assessment of sleep, leading to an underrecognized and undertreated condition. Provider failure in addressing sleep-wake disturbances can result in chronic issues with insomnia and has a negative impact on quality of life and cancer survivorship. Often sleep-wake disturbances present in symptom "clusters" including, anxiety, depression, and fatigue, which adds to the complexity of managing sleep disorders in oncology. Aggressive management strategies for managing underlying symptom burden from disease or medications effects is a priority.

RESUMEN

The global oncology nursing workforce is essential to achieving Sustainable Development Goals 3.4 (reduce non-communicable disease morbidity by a third by 2030) and 3.8 (universal health coverage). Unfortunately, challenges to a robust oncology nursing workforce include nursing shortages, recruitment barriers (eg, perceptions of a demanding specialty with complex care and hazardous work environments), and burnout. Innovative recruitment strategies, onboarding and continuing education programmes, occupational safety measures, and burnout prevention interventions are documented solutions. The long-term effect of COVID-19 on oncology care worldwide is unknown, but immediate therapy interruptions, workforce consequences, and threats to standard oncology nursing practice are addressed here. Retention of experienced oncology nurses is crucial for future cancer control in all countries and must be addressed, particularly in resource-constrained countries with few oncology nursing staff and continuing out-migration of nurses to resource-rich countries. As the cancer burden worldwide increases, the future of the oncology nursing workforce is reflected in the call from the International Council of Nurses, Nursing Now, and WHO for nurses to move to higher levels of leadership, advocacy, and policy making (ie, national cancer control planning) and assume responsibility for their key role in achieving global goals for cancer control.

OBJECTIVES: (1) To determine the perception of Compassion Satisfaction and Fatigue of oncology nursing staff; (2) to study the correlation between Compassion Satisfaction and Fatigue and sociodemographic, professional and adaptive variables (resilience, attitudes toward death, personality); and (3) to identify predictors of the two dimensions. METHOD: Descriptive, correlational, cross-sectional study. 69 nurses (62.7%) and 41 auxiliary care technicians (37.3%) from the Oncology Services in Biscay (Basque Health Service) completed the ProQOL-V, CD-RISC-10, DAP-R, and NEO-FFI-3. Data were collected between September 2018 and March 2019. The statistical analysis with SPSS.22 included chi-square tests, comparison of means, Pearson's correlation, and multivariate logistic regression. RESULTS: 66.4% (n=73) showed high Compassion Satisfaction, and 41.8% (n=46) were at moderate levels of Compassion Fatigue. People with prior studies related to death and/or grief were more satisfied. Compassion Satisfaction had stronger correlations with resilience and extroversion, and Compassion Fatigue did so with neuroticism and resilience. Four predictor variables emerged for Compassion Satisfaction: age, prior learning, resilience, and agreeableness; and four for Compassion Fatigue: attitudes of avoidance and escape concerning death, neuroticism, and openness. CONCLUSIONS: The high relationship found between training and resilience with Compassion Satisfaction can serve as a guide for academic and health care institutions to orient training, preventive and interventional strategies to provide oncology nursing teams with resources that allow them to optimize their care role

RESUMEN

The phenomenology of practice is a qualitative scientific method, defended by van Manen, who intends to analyze the phenomenon associated with one's lived experience. This article aims to analyze the application of the scientific method of the phenomenology of practice in the scientific production published in the oncology nursing discipline between 2010 and 2018, through a bibliometric analysis. As a method of analysis of the articles we resorted to bibliometric review because it allows a quantitative and objective reflection through the use of bibliometric indicators. As a result of the analysis of the 19 articles, 73% of the articles have as target population the cancer patient and their experience of the disease and its treatment. The publications are made in journals with an average IF of 1,337 and with the maximum number of citations of 54. As a conclusion of this bibliometric analysis, the fact that the method of phenomenology of practice has been increasingly applicable in Nursing stands out. Oncology in recent years, with relevance in the production of knowledge and which provides this discipline to illustrate the life experience of the people being studied

RESUMEN

Attendees at a 2019 ONS Congress session about disparities in clinical trials seemed to be aware that African Americans are underrepresented. But what about Hispanic patients with cancer? I sensed that some of my colleagues had not considered Hispanic recruitment before and were surprised to learn the facts about clinical trial disparities in this population.

RESUMEN

THEORETICAL PRINCIPLES: Pediatric oncology nurses are particularly vulnerable to emotional distress. Responsible for the oversight of a child's care, these nurses sustain close interactions with multiple patients and families over time, many of whom are coping with life-limiting diagnoses. The world of pediatric oncology nurses is one where tragedy is routinely witnessed thus demanding self-care and healing across a continuum. PHENOMENON ADDRESSED: The aim of this article is to outline and review the emotional sequelae of pediatric oncology nurses' work and to suggest interventions to support well-being in light of prolonged caregiving. Three major categories that are addressed include the aspects of clinical practice that influence caregiving, the risks of burnout, compassion fatigue, moral distress and grief, and interventions to counteract these phenomena. RESEARCH LINKAGES: Future-nursing research should focus upon the development of validated, psychometrically sound measurement tools to assess nurse-specific variants of burnout, compassion fatigue, moral distress, and nurse grief. Qualitative research should investigate the relationship between personal variables, workplace and team characteristics, age and experience, and their influence on the predominance of burnout, compassion fatigue, moral distress, and nurse grief. Lastly, the phenomena of resiliency demands further study.

Introduction. Going through the experience of having a child with cancer often results in a structural and emotional crisis for the family. Nurses, as professional members of a multidisciplinary team, interrelate the most with the families of oncologic pediatric patients. It is of great interest to provide evidence of nurses’ experience, in order to improve the care practice for children with cancer. Due to these patients’ highly complex needs in terms of care, a holistic approach is required. Objectives. To design a study which will determine the impact that the debut of an oncologic disease on a child has on families, by comparing nurses’ perception on the impact and experience of families going through this process to those of the actual family. Field of study. Oncology Unit and Day Hospital Sant Joan de Déu, Barcelona. Methodology. A qualitative study based on Heidegger’s hermeneutic phenomenology. The subjects of study will be parents of children with cancer and nurses involved in their care. For data collection, interviews and group discussions (if considered relevant) will be collected. The type of analysis used will be content analysis, following the QUAGOL Guide (Qualitative Analysis Guide of Leuven) (AU)

RESUMEN

Objective: The purpose of this study was to obtain an overview on nurses' opinions and perceptions related to pain assessment and other problems experienced in evaluating pain in cancer patients. Method: This descriptive qualitative study was part of a series of studies to develop a pain assessment tool for cancer patients in Indonesia. Data were collected by semi-structured interviews from 16 oncology nurses. A cross-case comparison of the interview responses was used to analyze the data from verbatim interview transcripts. Results: Six themes were identified: pain as the main complaint reported by cancer patients; pain assessment using a pain scale; an awareness of the absence of nurses' role in pain management; a lack of understanding of pain conditions; acknowledgement that a pain assessment tool should be practical and user friendly; and the need to develop a pain assessment tool. We also explored the knowledge of nurses in pain management and achieved an in-depth understanding of the nurses' desire to seek additional knowledge related to pain assessment. Conclusions: A pain assessment tool should be developed to assist nurses to independently interpret data about pain and allow them to select or modify interventions consistent with the patients' needs. Pain management for cancer patients is one of the essential nurses' responsibilities in providing comprehensive meaningful nursing care

RESUMEN

Purpose. The needs and concerns of patients with advanced breast cancer are changing at every phase of the care intervention. Management and coordination of hospital resources and services are also steadily evolving. The objective of the present expert report is to define a new oncology nursing role specialising in advanced breast cancer, to help guide patients throughout the whole healthcare itinerary. Methods. A group of eight experts in oncology nursing and medical oncology defined the content index of the curriculum document. A systematic review of bibliography was carried out, and the relevant contents were extracted. Based on these contents and the participants’ experience, recommendations were formulated and validated through a Delphi questionnaire and a participative meeting. Results. The advanced breast cancer clinical nurse (ABCCN) should develop a clinical, psychosocial role focused on coordinating patients in the healthcare network. The nurse would be in charge of evaluating and supervising the care administered and the healthcare resources used. The ABCCN should be aware and participate in the protocols and available resources, be able to solve conflicts, deal with burn-out signs and have clinical, coaching and team-working abilities. The proposed curriculum provides a specific process for the care of patients, as well as an implementation process. Conclusions. The ABCCN’s role is crucial to assume the best care and the optimisation of available resources. This review and consensus document provides the required tools for the implementation in hospitals (AU)

RESUMEN

The past three months have been a season of tremendous change, and many of us have been surprised by the political events that have taken place. January heralds a change in administration for the United States, and the future seems uncertain. I sense that change is going to be the new normal, and we have to be able to cope with it-and with each other-for a long time. What I want to talk about is those things that do not change and how we, as nurses and scientists, must remain true to our roots.
